I want to share the problem of my cousin. Just after 2 months of her marriage she has missed her periods. She had done pregnancy test at home that turned out positive. Unfortunately just after 3-4 days, she got severe pain in her vagina. By the next day she started to bleed from vagina. She visited to the doctor who has given her few medicines. Although doctor has give medicines for 5 days  but the doctor didn't told her clearly that what has happened with her. In actual, there is no expert doctor in this city. I want to ask that whether it is abortion, if yes then will it be cured with the medicines or she has to be undergone D and C since she is still having bleeding.

It is difficult to tell that what is the cause of this bleeding during pregnancy, since there could be a lot of reason of bleeding during pregnancy. I advice you to consult any good Gynecologist even though if you have to visit to the nearby city. After knowing the exact nature of bleeding, the proper treatment could be started. For your information, let us discuss briefly about the causes of bleeding during pregnancy.

Bleeding In Pregnancy – Early Pregnancy – Late Pregnancy: Ante-Partum Hemorrhage

Bleeding in pregnancy could be due to several reasons and also its amount varies from slight spotting till heavy bleeding. If few spots arrives at any time of pregnancy then it should not be alarming. If spotting would happen frequently and along with it bleeding in large amount would happen then it shows some thing is there considerably wrong. Bleeding During Pregnancy at any month is called medically as Ante-Partum Hemorrhage.

Bleeding During Pregnancy (Early Or Late Pregnancy) – Placenta Previa:

Placenta is a cord like structure, which is attached with the umbilicus of the fetus and connects the inner blood vessels of the fetus to the blood vessels of the mother on the uterus. The function of this placenta is to provide nutrients, oxygen and all necessary ingredients of life to the baby from mother. If the placental position is abnormally low and near the os of uterus, then it is called as Placenta Previa. In Placenta Previa, bleeding ranging from minute spotting to the heavy bleeding may occur. The Ultrasound examination would reveal exactly the position of placenta Previa. Usually the bleeding in pregnancy due to Placenta Previa may happen in early pregnancy, however; this is not the rule and it may occur at any time during pregnancy. Usually the Gynecologist advised the mother to take bed rest and calls her regularly for her check up at fixed interval of time. She keeps on observing the fetal viability and mother's health status. In some cases however; if the condition would become more serious then abortion could be advised for the sake of mother health. In few cases, miscarriage could also happen. The best treatment and strict rest could vanish this problem however.

Bleeding During Pregnancy (Early Or Late Pregnancy) – Abrupto Placenta:

In this condition, the placenta would get tear in its body and develops bleeding in pregnancy. This condition is more serious and the patient has to be shifted in to the hospital. If the tear would be un-repairable, then the operation could be done to evacuate the baby and hence abortion could be proceeded. Another problem is that a patient who develops Abrupto Placenta might get ARDS (Acute Respiratory Distress Syndrome) which is a life threatening issue. In this case, the patient has to be shifted to the ICU. In Intensive care, the patient may be ventilated mechanically. Pregnancy it self a hyper dynamic and hyper metabolic state and therefore; some times the condition is overlooked and misdiagnosed as ARDS, however; the clinician should consider it as well while evaluating the case of Abrupto Placenta.  

Bleeding During Pregnancy (Early Or Late Pregnancy) – Pregnancy Bleeding After Intercourse

Post-coital bleeding or bleeding in pregnancy after intercourse should be evaluated at prompt by consulting any good doctor. In most of the cases, it might happen due to the low lie of placenta not coming although in the category of Placenta Previa. This condition however; is not serious but the immediate evaluation should be done in order to decrease the chances of any mis-fortune events.

Bleeding could happen in the early phase of pregnancy or in first trimester due to abortion or miscarriage. There are various causes of this morbidity. The woman could feel pain as well in her tummy followed by the bleeding from vagina. The bleeding could be either fresh or clotted and mixed with the pieces of product of conception. Urgent medical advice and treatment should be sought in this condition.

Bleeding During Pregnancy (Early Or Late Pregnancy) - Ectopic Pregnancy

Ectopic Pregnancy means the pregnancy held out side the uterus, mainly in the fallopian tubes. Some times the pregnancy may happen inside the abdomen cavity or peritoneum. A woman may experience sharp pain and bleeding from vagina in case of Ectopic pregnancy. This condition would have more chances to develop if there have been previous pelvic surgeries done, the woman does have the history of Ectopic pregnancy and there is infection in the fallopian tubes. The woman does have low level of B-HCG in case of Ectopic pregnancy compared to the normal pregnancy.

Bleeding During Pregnancy (Early Or Late Pregnancy) – Molar Pregnancy:

Although rare but still could be the cause of bleeding during pregnancy. The B-HCG levels would be found high, the fetal heart sound would be absent and the grape like structure would be found in uterus on ultrasound.

Bleeding During Pregnancy (Early Or Late Pregnancy) – Implantation Bleeding:

Whenever the zygote is implanted in the uterus, the minute amount of bleeding or spotting could happen very early in the pregnancy. This is called as implantation bleeding. This is not harmful.

Bleeding During Pregnancy (Early Or Late Pregnancy) – Preterm Labor:

Vaginal bleeding could happen due  to labor mainly preterm labor. When the pregnancy is about to end earlier than the due date and when the bleeding starts before 37 weeks of pregnancy, the woman should immediately reach to the hospital where she would be observed for quite some time and the appropriate actions would be taken accordingly.

Bleeding During Pregnancy (Early Or Late Pregnancy) – Periods During Pregnancy:

About 10% of all women experience bleeding which is light in amount at the time of their periods every month during pregnancy. This bleeding is harmless and should not be taken in to serious consideration unless it would become heavy or happen in late pregnancy.

Bleeding During Pregnancy (Early Or Late Pregnancy) – Cervical Ectropion

Certain harmless changes happen in the cervix during late pregnancy and that would generate bleeding. This is again not alarming.

Bleeding During Pregnancy (Early Or Late Pregnancy) – How To tackle:

If a woman experiences bleeding during pregnancy then the cause must be evaluated in order to get safety from any jeopardy

If the bleeding would happen just after intercourse, then abstinence from intercourse for quite some times is required before the cause of bleeding would not be confirmed.

Early and prompt medical advice should be sought so that possibility of infection and any other complication of pregnancy would be prevented.

Ultrasonograpy would be advised in most of the case and found quite helpful.

There is nothing to worry about in the cases of bleeding per vagina during pregnancy since in majority of cases, the bleeding is not harmful.

There are certain other causes of bleeding beside gynecological one and that has to be evaluated as well.
